Appointment Policies

We believe it is important that patients keep their regularly scheduled appointments so that dental care is provided in a timely fashion to maintain ideal dental health. Our staff expects you to keep your scheduled appointments unless unusual circumstances arise. We require a minimum of 24-hour’s notice to cancel any dental appointments. A non-refundable scheduling deposit may be required prior to making an appointment if you frequently fail to show or cancel your dental appointments. In the event of frequent cancellations or several missed appointments, you may be dismissed from treatment at Dahm Dental.

If you have several family members in your family we typically schedule only two members per day.


Payment Policies

Dahm Dental expects payment at the time of service. For patients with Delta Dental insurance or other insurances that pay directly to Dahm Dental, your estimated co-insurance is expected on the date of your dental appointment. For insurance companies who reimburse the patient directly, payment in full for dental services is expected at the time of your appointment.  We accept Visa, MasterCard, Discover, American Express, personal checks and cash.

Dahm Dental has several financing options available which include the acceptance of cash, check, debit and credit cards, AdvanceCare Dental and Care Credit (3rd party financing companies), ACH auto debit withdrawals, pre-payment plans and pre-paid gift cards for large treatment plans that are paid in full before service is rendered. Please ask to speak with our Financial Coordinator to discuss a personalized plan to meet your individual needs.


Insurance Policies

Dahm Dental will file your dental insurance claims with your dental insurance company as a courtesy to our patients whether we are a preferred provider for your insurance policy or not. We are a preferred provider for most Delta Dental insurances; however, we are not a preferred provider for any other dental insurance company. We can still file to our dental insurance company, but it will be considered out-of-network benefits. Upon receipt of your services, we will provide you with an estimation of your co-insurance due. Payment for services is due at the time of your appointment.  In some instances, the dental insurance company will pay you, the subscriber.  Hence, payment will be do in full at the time of service.  

  


Charity Care/Financial Hardship

Dahm Dental is committed to serving dental patients around the Pella area regardless of a patient's ability to pay for such services.  Patients whose health insurance coverage, if any, does not provide full coverage for all of their dental expenses and whose dental expenses, in relationship to their income, would make them indigent if they were forced to pay full charges for their dental expenses may request free or discounted services from Dahm Dental.  Dahm Dental will make decisions related to the provision of charity care in a manner that balances community and patient needs with the necessity that the practice continue as a successful business.


We are not a provider for the Dental Wellness Program through Delta Dental and MCNA as well as Wellmark Federal Employee Basic Option 111 or 112 plans; therefore, services rendered under these two insurance programs, patients will the responsible for full payment.
